We have moved to an out of network status with Blue Cross/Anthem at the end of 2021 and with United Healthcare (UHC, GEHA, Secure Horizons) on March 4, 2022. While we are reluctant to do this and realize the impact on our clients, we have had to make this decision. We are happy to answer any questions you might have.

Medicare and all secondary insurances filed through Medicare, however, at this time we are not accepting new Medicare patients.
Though Functional Physical Therapy is out of network for all insurance companies except Medicare, we are happy to provide you with the information necessary for you to your visit to your insurer for their consideration for payment
